This Testosterone Replacement Therapy (TRT) Consent describes the risks, benefits, limitations, and responsibilities associated with receiving TRT services through Jack Business Management Inc DBA VARO (“VARO,” “we,” “our,” or “us”).
By proceeding with TRT evaluation or treatment, you acknowledge and agree to the terms outlined in this Consent.
TRT may be used to address symptoms associated with low testosterone levels, as determined by a licensed clinician.
TRT is not appropriate for everyone, and eligibility is determined through clinical evaluation, medical history review, and laboratory testing.
Potential benefits of TRT may include:
Improved energy
Enhanced mood
Increased libido
Improved sexual function
Increased muscle mass
Reduced fat mass
Improved sense of well‑being
Individual results vary, and benefits are not guaranteed.
TRT carries risks, including but not limited to:
Acne or oily skin
Hair loss
Mood changes
Sleep disturbances
Elevated red blood cell count
Changes in cholesterol
Fluid retention
Fertility reduction
Prostate enlargement
Worsening of sleep apnea
Injection‑site reactions (if applicable)
Your clinician will discuss risks relevant to your situation. If you experience concerning symptoms, seek medical attention promptly.
TRT requires ongoing monitoring, which may include:
Blood tests
Hormone level checks
Cardiovascular risk assessments
Prostate health evaluations
Follow‑up telehealth visits
You agree to comply with recommended monitoring to ensure safe treatment.
TRT consultations may occur via telehealth. You acknowledge that telehealth:
May limit physical examination
May require additional in‑person testing
May require external lab work
Is not appropriate for emergencies
If you experience an emergency, call 911 immediately.
VARO does not dispense prescription medications.
If a VARO clinician determines TRT is appropriate:
A prescription may be issued
It will be sent to an external licensed pharmacy
VARO is not responsible for pharmacy fulfillment
Controlled substances are not prescribed
Medication availability and pricing may vary
You must disclose all medications, supplements, and medical conditions to your clinician.
You understand that TRT may be denied or discontinued if:
Lab results do not support treatment
You fail to complete required testing
You provide inaccurate or incomplete information
TRT is deemed medically inappropriate
You violate program policies
You agree to:
Provide accurate medical information
Complete required lab testing
Attend follow‑up appointments
Report side effects or concerns
Follow clinician guidance
Use medications only as prescribed
Avoid sharing or misusing medications
You understand that:
TRT consultations may incur fees
Lab testing may have separate costs
TRT prescriptions are billed by external pharmacies
Fees for completed services are non‑refundable
Program fees may be non‑refundable once initiated

Allow transmission of PHI electronically
