This Telehealth Consent describes how telehealth services are delivered by Jack Business Management Inc DBA VARO (“VARO,” “we,” “our,” or “us”).
By using VARO’s telehealth services, you acknowledge and agree to the terms outlined in this Consent.
Telehealth involves the use of electronic communications to enable healthcare providers to deliver clinical services remotely. These communications may include:
Video conferencing
Audio calls
Secure messaging
Digital images
Remote monitoring
Online assessments
Telehealth allows you to receive care without an in‑person visit.
Telehealth may offer:
Increased access to care
Convenience and reduced travel time
Faster scheduling
Continuity of care
Access to specialized providers
Telehealth has limitations and risks, including:
Technical failures or poor connection
Delays due to equipment or internet issues
Incomplete information due to lack of physical examination
Security breaches despite safeguards
Miscommunication due to technology limitations
If a provider determines telehealth is not appropriate, you may be referred for in‑person care.
You agree to:
Provide accurate and complete information
Use a private, secure location for sessions
Ensure your device and internet connection are functioning
Follow provider instructions
Disclose all medications, allergies, and medical conditions
Seek emergency care when needed
Telehealth is not appropriate for emergencies. If you are experiencing an emergency, call 911 immediately.
VARO clinicians will:
Determine whether telehealth is appropriate
Provide care consistent with professional standards
Protect your privacy and PHI
Document your visit in your medical record
Inform you of treatment options, risks, and alternatives
VARO uses HIPAA‑compliant platforms and encryption to protect your information.
However, you acknowledge that:
No system is 100% secure
Electronic communication may be intercepted or disrupted
You are responsible for securing your own device and network
Your privacy is protected under:
HIPAA
HITECH
Applicable state laws
Telehealth sessions may involve transmission of PHI. VARO will not record telehealth sessions without your consent.

Telehealth may not allow providers to:
Perform a full physical exam
Detect certain conditions
Provide certain treatments
Prescribe certain medications
Providers may require:
Lab testing
Imaging
In‑person evaluation
Follow‑up visits
VARO does not dispense prescription medications.
If a VARO clinician issues a prescription:
It will be sent to an external licensed pharmacy
VARO is not responsible for pharmacy fulfillment
Controlled substances are not prescribed
Medication availability and pricing may vary
By using VARO’s telehealth services, you consent to:
Receive care via telehealth
Allow providers to review your medical information
Allow transmission of PHI electronically
Participate in remote assessments
Follow provider recommendations
Use external labs or pharmacies when required
You may withdraw consent at any time by discontinuing telehealth services.
You understand that:
Telehealth services may incur fees
Fees must be paid at the time of service
Telehealth consultations are non‑refundable once completed
Additional services (labs, prescriptions, products) may have separate costs
